Lab Requests

Please use thePathology/Cytology Submission form (Form #2) when submitting biopsy or cytology specimens for testing. For Chemistry, Hematology, Microbiology, and most other specimens, please use the Chemistry/Micro Submission form (Form #1).
Shipping supplies, submission forms, airbills and supplies needed for tests performed by HPA (except blood tubes and forms for Coggins) are provided to our clients free of charge.

Packaging & Shipping

Courier service is provided through FedEx Next Day Air service at no additional charge for most specimens. Please request our price list for details. We can provide you with preprinted airbills for your convenience. To schedule pickup from your clinic, call 1-800-GOFEDEX, or take your package to any authorized FedEx outlet or give it to any FedEx driver.

Coggins Tests can also be submitted using globalvetlink.com. If you use this service, please remember to print the online Lab Submittal Form and include it with your specimen tubes.
